Accessing your funds to pay for qualified medical expenses is easy with a MasterCard debit card (medical and limited only)

Plan runs on a calendar year with an ‘Extended Grace Period’ through March 15th of the following year – you have until March 15th of the following year to incur claims

  • Deadline to file claims is April 15th following the end of the plan year (12/31) or funds will be forfeited

Provides term life insurance lump sum payment:

For You: Up to 5 times your salary, to a max of $750,000, in $10,000 increments

For Your Spouse: 100% of your coverage, up to $100,000, in $10,000 increments

For Your Children: $10,000 for each child

Group Critical Illness insurance pays cash benefits if you or a covered dependent is diagnosed with a covered condition. Benefits are paid directly to you, unless assigned, and can be used in any way you see fit.

Choice of 2 benefit levels

Group Accident Insurance pays cash benefits if you are injured in a covered accident.

Group Hospital Indemnity plan pays cash benefits directly to you, if you or a covered dependent is hospitalized due to illness or injury.

AFLAC SHORT-TERM DISABILITY INSURANCE

Group disability insurance pays cash benefits when you are unable to work due to covered off-job accident or illness. Benefits are paid directly to you, unless assigned, and can be used in any way you see fit. Benefits include:

Guaranteed Issue coverage so you don’t have to answer any health questions

Pays up to 60% of your salary (up to 40% in states with state disability)

Partial disability benefit

3-month or 6-month coverage options
